Type CV2013 is a CV2127 with tab leads welded to the base pins. The underlying design is the 6CH6 a video output amplifier used in professional applications for driving TV or radar signals down (fairly) long cables.
The valve was made by Brimar in England.
A closer view of the two inner grids that pass flat across the cathode. Any secondary emission electrons are prevented from reaching the screen grid by the beam plates.
The base pins with the added tapes welded to the original pins. Note the added central mock spigot that also has a tape attached.
The thin glass tube envelope is 20 mm in diameter, and excluding the B9A base pins, is 60 mm tall.
