The 12AZ7A is a double triode designed for audio voltage amplification and/or as an oscillator and mixer in VHF television receivers.
This exhibit is an original General Electric product from the 1950s or 1960s.
The ratings are very similar to the 12AT7 or ECC81 double triodes.

One of the triode sections in close-up. Note the copper grid supports and the stitched anode. The cathode tubes are round in cross section.
The thin glass tube envelope is 20 mm in diameter, and excluding the B9A base pins is 47 mm tall.
