The Mazda V111 is a television line output pentode. The former service name was the VT127 and later it became the CV1127

Here the control grid can be seen close to the cathode. The valve does not have aligned grids and the pitch of the control grid is smaller than the screen grid.
The classic envelope is 53 mm in diameter, and excluding the MO base pins is 110 mm tall.
