The CV3612 (commercially the 5686) is a special quality pentode. It can be used as a Class A audio amplifier to give 2.7 Watts single ended or a Class C RF amplifier up to 160 MHz where the output is just over five Watts.
The original Type designation on the envelop can be seen on the left. Overprinted in green is the GE 5686 and Made in USA. The final overprint is the UK up arrow and the CV designation.

The inner electrodes are hidden but the anode and the screens top and bottom. At the base the copper control grid support is visible.
The thin glass tube envelope is 21 mm in diameter, and excluding the B9A base pins is 47 mm tall.
