The NR94 is an indirectly heated triode based on the AD/P4. The design purpose was for line scanning in TV circuits. This valve is marked with the old Navy Receiving code of NR94. Later it was renamed the CV1198.
CV1190 refers to a pair of CV1198s selected such that with 1.0 kV on the anode, with a 100kΩ anode load and an anode current of 4.75 mA, the control grid voltages are within half a volt of each other.
The triode looks to be only 4 mm front to back.

The classic envelope is 39 mm in diameter and, excluding the B5 base pins, is 110 mm tall.
