The CV66 is an early all glass valve in an aluminium screening can, was designed as an RF amplifier operating in grounded grid configuration. The multiple connection points are to reduce lead inductance, and thereby increase the useful operating frequency.
The CV66 was originally known as the RL37. When issued commercially it was given the Type designation EC54.

The reverse of the can is left plain.
The wide glass tube envelope is 33 mm in diameter and, excluding the B9G base pins, is 60 mm tall.
